Prepayment Penalty
Check this box if the Original loan had a prepayment penalty.

Prepayment Term
If the Original loan has a prepayment penalty, input the term during which the original prepayment penalty applies. Any number between 1 and 5 refers to years. Numbers 6 and greater refer to months.

Loan Amount
Input the total Loan Amount of the Original loan, including any financed mortgage insurance (MI) or VA funding fee.

Recording Instrument No.
Input the Instrument Number for the Original loan.

Outstanding Balance
Input the remaining principal balance of the original loan.

Note Date
Input the Note Date for the original loan in an MM/DD/YYYY format.

Loan Lock Date
Input the Lock Date for the original loan in an MM/DD/YYYY format.

Recording Date
Input the Recording Date for the original loan in an MM/DD/YYYY format.

Original Completion Date
Input the Original Completion Date for the original loan in an MM/DD/YYYY format.

Max Draw
Input the maximum percentage amount that the unpaid principal balance can never exceed. This value should be over 100%.

Recording Book Number
Input the liber number where the document is recorded. Liber is the term used in some states instead of using the term "book" and "page" when referring to the recording information.

Official Recorder of Instrument
Input the name of the state government entity that fulfills the role of "official recorder of instruments".
For Example: Kanawha County, West Virginia - Clerk/Property Records

Recording Page Number
Input the land record's page number where the loan to be paid off or assigned was recorded.

Land Records Office
Input the name of the land record's office. This information will vary by state and can be gathered from the recording stamp and/or seal that is attached to the face of the original recorded instrument.

Recordable Transaction
Check this box if the loan modification package will be recorded.

Stated Balloon Payment
If applicable, the dollar amount of the Stated Balloon Payment that will be tacked onto the end of the loan modification transaction. (Note: this amount will not be subject to interest charges.)
