Seller Name
Input the seller's first name and, if applicable, middle initial, in the First Name field. Type the seller's last name in the Last Name field. There are fields for two sellers. If the seller is a corporation, input the corporation name in the field for the first name, and leave the last name field blank.
Seller Address
Use the two lines provided to input the address of the seller, including their city, state (two-letter abbreviation), and zip code on the second line. This information may not always be available.
Concessions
Please input the concession amount, if applicable, and the percent in which it is not to exceed.
