There are up to four completed service provider sections and three blank sections. The four completed providers are:
- Tax Services
- Consumer Credit Report Services
- Property Appraisal Services
- Flood Zone Determination Services
Each of the four completed provider sections is preset with the corresponding HUD number. The three blank provider spaces may be used for any other fees that list on the Fees screen.
