Initial Advance
Input the initial advance amount taken when the loan is opened.

Minimum Initial Draw
Input the minimum credit advance that the borrower can receive when the initial draw is taken.

Minimum Draw
Input the minimum credit advance that the borrower can receive at any given time (not including the initial draw).

Draw Period
Input the period of time during which the borrower can obtain advances of credit.

Draw Period Renewal
Input the number of times the Draw Period may be renewed based upon a set number of year or month periods. For example: "The Draw Period may be renewed for up to _____ additional five year periods."

Repayment Period
Input the period of time during which the borrower must repay the outstanding loan balance, with accrued interest, but may not request further advances. The repayment period begins at the end of the Draw Period.

Index Description
Enter a brief description of the type of index used for the loan. (e.g., 1 Yr LIBOR, 11th District Cost of Funds Index)

Index
Input the current index rate for this loan.

Margin
Input the percentage rate of the margin for this loan.

Arbitration State Law
If the HELOC Agreement contains a mandatory arbitration clause, then indicate the state whose arbitration laws will apply in an arbitration to resolve a dispute regarding this HELOC Agreement.

Arbitration City/State
Input the city and state location where the arbitration will take place.

Teaser Rate
Input the low introductory interest rate available for a few months at the start of the loan.

Teaser Term
Input the number of monthly payments for which the Teaser Rate will be in effect.

Teaser Margin
Enter the percentage rate of the margin during the Teaser Term.

Program Type
Select a Program Type from the pull-down menu. Your options for Program Type may include:
  • Full Cost
  • No Cost
Maximum APR
Input the maximum Annual Percentage Rate that can be reached for the life of the loan.

Minimum APR
Input the minimum Annual Percentage Rate that can be in effect at any time during the life of the loan.

Piggy Back
Checkmark this box to indicate that this HELOC transaction is made simultaneously with a purchase money loan/mortgage and that both such loans are made for the purpose of acquiring the Property.

First Loan Balance
Input the unpaid balance of the first mortgage.

Third Party Fees Less Than
Enter the actual or best estimate of the minimum fees paid to - or to be paid to - third parties related to this transaction.

Third Party Fees More Than
Enter the actual or best estimate of the maximum fees paid to - or to be paid to - third parties related to this transaction.
